1. Nature of the Program

The ReGenesis Longevity Program is a personalized, preventative, and optimization-focused wellness and healthcare service designed to support improved healthspan through advanced diagnostics, clinical oversight, and lifestyle interventions.

The Program may include, but is not limited to:

• Comprehensive health assessments
• Advanced laboratory testing and biomarker analysis
• Epigenetic testing
• Imaging and diagnostics (where applicable)
• Individualized care planning
• Hormone optimization and regenerative therapies (when clinically appropriate)
• Nutrition, fitness, and lifestyle education
• Ongoing monitoring and progress reviews

The Program is not intended to replace emergency medical services or primary care. Participants are encouraged to maintain a relationship with their primary healthcare provider.

2. No Guarantee of Outcomes

While ReGenesis utilizes evidence-informed protocols and advanced diagnostics, individual outcomes vary.

You acknowledge and agree that:

• Results are not guaranteed
• Longevity optimization is not an exact science
• Improvements in health markers or quality of life cannot be promised
• Responses to treatments and interventions differ between individuals

ReGenesis does not warrant or guarantee specific medical, physical, or longevity outcomes.

3. Medical Disclaimer

Participation in the Program does not constitute a guarantee of disease prevention, reversal, or cure.

All recommendations are provided based on available clinical information and current scientific understanding. You understand that medical science evolves and protocols may change accordingly.

You agree to disclose complete and accurate medical information and to notify ReGenesis of any changes to your health status.

4. Eligibility and Participation

By enrolling, you confirm that:

• You are at least 18 years of age
• You are legally capable of consenting to healthcare services
• You will provide truthful and complete health information
• You will follow clinical guidance as recommended

ReGenesis reserves the right to decline or discontinue services if participation is deemed clinically inappropriate or unsafe.

5. Fees, Payment, and Program Structure

The ReGenesis Longevity Program is a private, fee-for-service offering and is not covered under Alberta Health Care Insurance Plan.

Fees may include:

• Initial assessments
• Diagnostic testing
• Clinical consultations
• Program memberships
• Treatments and therapies
• Supplement or pharmaceutical costs

Payment terms will be outlined in your individual service agreement.

Unless otherwise stated:

• Fees are payable in advance
• Diagnostic and lab fees are non-refundable once ordered
• Missed appointments may be subject to cancellation fees
• Program fees are non-transferable

Refund policies, if applicable, will be provided separately.

6. Third-Party Providers

Your care may involve third-party partners such as laboratories, imaging providers, pharmacies, or digital health platforms.

While ReGenesis carefully selects its partners, we are not responsible for services provided directly by third parties. Each third party operates under its own terms, policies, and professional obligations.

7. Use of Digital Platforms

Participation in the Program may involve electronic health records, secure portals, apps, or remote communication tools.

You acknowledge that:

• Digital communication carries inherent risks
• ReGenesis employs reasonable safeguards but cannot guarantee absolute security
• You are responsible for protecting your login credentials

8. Privacy

Your personal and health information is handled in accordance with our Privacy Policy and applicable Alberta and Canadian privacy legislation.

By participating, you consent to the collection, use, and disclosure of your information as outlined in our Privacy Policy.

9. Limitation of Liability

To the fullest extent permitted by law:

• ReGenesis shall not be liable for indirect, incidental, or consequential damages
• Liability is limited to the amount paid by you for Program services
• Participation is voluntary and undertaken at your own informed discretion

10. Termination of Services

You may withdraw from the Program at any time by providing written notice.

ReGenesis may suspend or terminate services if:

• Terms are breached
• Payment obligations are unmet
• Continued participation is clinically inappropriate
• There is abusive, unsafe, or disruptive behavior

Termination does not eliminate outstanding financial obligations.

11. Intellectual Property

All Program materials, protocols, content, and educational resources remain the intellectual property of ReGenesis and may not be reproduced or distributed without written consent.

12. Governing Law

These Terms are governed by the laws of the Province of Alberta and the laws of Canada applicable therein.

Any disputes shall be resolved in Alberta courts.

13. Amendments

ReGenesis reserves the right to update these Terms at any time. The most current version will be made available through our clinic or website.

Continued participation constitutes acceptance of updated Terms.
